Instructions:
1. All listed JO1 applicants are instructed to download the BJMPRO-NCR Medical Laboratory Request & Dental Requirements, proceed to any DOH-accredited Laboratory, and attach results to your MEDICAL EXAMINATION RECORD FORM. Submit all requirements during your scheduled reporting. Ensure to photocopy all laboratory results prior to scheduled date.

✅ Fasting (at least 10hrs) No intake of food and fluid.

✅Medical Certificate CS FORM NO.211 revised 2018 (A4 paper size) with extra copy
