A new way of caring for people with long-term conditions has opened at Haslemere Hospital 🩵
The Haslemere Proactive Care Hub (PACH) brings specialist care together in a single visit, helping people with long-term conditions stay well and independent for longer.
Based in space previously used for inpatient wards, the hub is working with local GPs to invite patients in for a comprehensive health assessment. The aim is to spot small changes early, before they become more serious and harder to treat.
Initially focused on heart failure, the hub runs one day a week. In one two-to-three-hour appointment, patients can see:
❤️ A cardiologist to assess heart health
🚶 A therapist to assess strength and falls risk
💊 A GP-led primary care team for medication reviews and general health checks
Together, the team creates a personalised care plan. A social prescriber will join as the service develops, connecting patients with wider support in their community.
Dr Simon Vickers, Royal Surrey Clinical Director for Proactive Care and a Guildford-based GP, said: “It’s a proactive approach to treating patients, drawing on a range of expertise to provide holistic care. It bolsters the service provided by GPs and will help us identify and address signs of declining health at an earlier stage, when interventions are typically more successful.”
The hub plans to expand to other long-term conditions, shaped by local need, from early 2027. Once the Haslemere model is established, we hope to replicate it across other communities in West Surrey.
The service supports the government’s 10 Year Health Plan for England, with its focus on earlier intervention, prevention and bringing care closer to home.
