We are looking for local Aged Care Care Workers.

SMB Home Care Assist is growing its team across Russell, Macleay, Lamb and Karragarra Islands.

We support older people to live safely and independently at home. The work can include personal care, social support, domestic assistance, meal preparation and help with everyday tasks.

We would like to hear from people are reliable, kind and comfortable working in clients' homes. Experience in aged care and dementia is welcome.

Clinical support is an important part of helping older people remain safe and independent at home.

Our clinical capacity includes:
* Experienced Registered Nurses providing nursing care, clinical assessment, monitoring and oversight.
* Coordination with allied health professionals, including physiotherapists, occupational therapists, exercise physiologists, dieticians and speech pathologists.
* Medication assistance based on each client's assessed needs and care plan.
* Clear communication when health needs change or concerns need to be escalated.

Our local care and clinical teams work together so clients' receive coordinated support from people who understand their needs.

Services are provided where included in the client's Support at Home care plan and funding arrangements.
