Join our team: SOAHAC is currently seeking the services of an Integrated Care Manager to join our team to work at our London site.
This role supports day-to-day operations, team development and coordination of interdisciplinary health and wellness services at SOAHAC’s London site. The Integrated Care Manager works closely with Traditional Healing teams, physicians, nurse practitioners, mental health and addictions workers, allied health professionals and community partners to support integrated care pathways grounded in cultural safety, relational practice and Indigenous wellness approaches.
$93,928 – $117,410 annual salary
35-hour work week
