The following applications will be accepted:
✅Registration
✅Transfer from Other City/Municipality and within the municipality.
✅Change/correction of Entries in the Registration Records
✅Reactivation of Registration Records
✅Inclusion of Registration Records

Any of the following IDs must be presented:
✅NBI
✅Postal ID
✅PWD ID
✅Student’s ID or Library card
✅Driver’s License
✅NBI Clearance
✅Philippine Passport
✅SSS ID/GSIS/UMID ID
✅PRC ID
✅NCIP
✅Any other Government issued valid IDs

and must include the complete address of the place where you wish to register/transfer

The following may also be presented during registration/transfer such as PROOF OF BILLING (electricity bill, water bill, internet, credit card statements, etc. ) named after you.

📍 CHANGE/CORRECTION OF ENTRIES
The following documents must be from PSA or not a Certified True Copy from the Local Civil Registrar.

✅ Birth Certificate - when there is an error in the name, date of birth, place of birth, or typographical errors

✅ Marriage Contract/Court Order - when there is a change in the name because married or validity of the court order

✅ CENOMAR - when changing status from "married" to "single"

Magandang araw po sa lahat ng taga-Ibayo.

Ako po ay nais lamang magbigay ng mas malinaw na paliwanag kaugnay sa mga kumakalat na usapin tungkol sa budget ng SK. Bilang SK Chairman ng Ibayo, mahalaga po sa akin na maging tapat at transparent sa ating mga kabataan at buong komunidad.

Sa kasalukuyan po, wala pa pong opisyal na budget na naibibigay o nalalabas. Ang karamihan po ng nagiging gastos para sa mga aktibidad at paghahanda ay mula pa rin sa personal na bulsa ng aming pamilya. Nais ko pong linawin ito dahil may mga haka-haka na tila may pondong ginagamit para sa pansariling interes, na hindi naman po totoo.

Kaya po ako nagpost at patuloy na magbibigay ng updates ay upang ipaalam sa lahat kung ano talaga ang nangyayari. Layunin ko pong itigil ang pagkalat ng maling impormasyon at mas maunawaan ng bawat isa ang tunay na sitwasyon ng ating SK.

Ako rin po ay humihingi ng inyong pang-unawa at pasensya. Tao lang din po ako na nagsisikap gawin ang aking tungkulin sa kabila ng mga limitasyon na aming kinakaharap.

Makakaasa po kayo na sa oras na may malinaw at opisyal nang update tungkol sa budget, ito po ay agad naming ipapaabot sa inyo.
