Abraxas Home Care, LLC is seeking a compassionate, dependable, and organized LPN to support individuals in residential and community settings.

🕗 Hours: 8:00 AM–4:00 PM
💵 Starting pay: $32 per hour up to $38 an hour depending on experience.

Responsibilities include:

• Coordinating medical appointments and follow-up care
• Supporting medication management and monitoring health needs
• Communicating with providers, families, guardians, and support teams
• Maintaining accurate nursing documentation
• Working with our RN and clinical team to support individualized care

Qualifications: Current Virginia LPN license, strong communication and organizational skills, and a commitment to person-centered care.
